To open a GROUPCARE scheme, you need at least half (minimum two), or more, of the dentists at your practice to be DDU members (excludes foundation dentists).

In this form you will be asked to provide details of all the DDU members in your practice. We need these details so that we can add them to your scheme and for them to get their membership discounts.
