Please state the total number of additional OOH or extended access sessions to be worked between 1 October 2018 and 31 March 2019 below. The sessions requested should not exceed six sessions on average per week over the duration of the scheme.

If the provider(s), or number of sessions, changes after you have submitted this form, you will need to submit a new form. New forms must contain the details of all the providers and sessions you have agreed to, not just additional or amended work.The total number of sessions across all providers cannot be more than 152.

An MDU session is defined as a period of time ‘up to four hours’ in length, e.g. a six hour clinic should be recorded as two sessions.
You can add extended access hours together to form a session, e.g. 8 x 1.5 hour mid week periods is 3 sessions. 
Is this work for out of hours or extended access sessions?
The criteria for extended access work would be that for the vast majority of consultations:
  • Pre-booked patients (can be same-day appointments) from the local area
  • Access to full GP records
  • Case mix of patients in line with routine in-hours GP care
  • Any referrals from NHS 111 or other services are screened to exclude emergencies
